Your role will involve investigating treatment history, conducting patient interviews, securing necessary information, and providing it to family and collaterals as appropriate. You will be involved with the management of clinical information including diagnostic impressions, as well as the recommended disposition for the communication to the ED physician and supervising psychiatrist. Lastly, you will take ownership of ensuring that this contact is documented according to standards set forth for your role and the department. Some highlights about this opportunity include: * Fantastic opportunity for Registered Nurses to further their experience in providing consultations in a behavioral health setting * Great opportunity for LMSWs and LCSWs to practice in an acute care setting * Opportunity to get refine your skills in incorporating biopsychosocial care approach for patients with a comorbid diagnostic profile * Opportunity to serve as vital member of a treatment team, viewing the patient from every perspective, and developing a resource/education structure for all stakeholders involved in patient care * * *The Glens Falls Hospital Impact* *Mission* * * Our Mission is to improve the health of people in our region by providing access to exceptional, affordable, and patient-centered care every day and in every setting. * * *How You Will Fulfill Your Potential* *Responsibilities* * Responds within 60 minutes to ECC requests for consultation * Reviews prior clinical records as available * Conducts evaluations of each referred ECC patient, according to policies and procedures * Assigns provisional psychiatric diagnosis and develops proposed disposition plans * Efficiently summarizes and presents case information to supervising psychiatrist and ECC physician * *Qualifications/Experience* *Education/Accredited Programs * o + **Bachelor's degree in social work or related field required + Minimum one year of experience in a behavioral health setting, or + Current license as registered professional nurse (RN) in New York with a minimum of three years of experience in psychiatric nursing + Master's degree in social work or related field strongly preferred * Experience/Abilities* o + ** 3 years’ experience in a behavioral health setting + OPWDD experience or training highly desired * Licenses/Certificates/Registrations:* o + NYS Licensed LMSW, LCSW, LCSW-R orNYS Licensed RN * Skills/Abilities* o + **Thorough knowledge of psychiatric diagnoses + Possesses the skills and knowledge to utilize therapeutic communication techniques to offer short-term crisis resolution counseling and de-escalation of crisis situations + Familiarity with psychiatric medication + Knowledge of insurance reimbursement issues and processes + Ability to conduct clinical patient interviews, including assessment of mental status + Ability to communicate clearly in written and verbal forms + Ability to interact with various psychiatric and mental health disciplines + Ability to manage time effectively and to cope with multiple simultaneous demands + Ability to respond calmly in crisis situations + High level perceptual and reasoning skills to allow for accurate assessment and determination of proper dispositions** ***Communities We Serve* Located in the foothills of the beautiful Adirondack mountains, Glens Falls is conveniently located a short drive away from the capital region and Lake George.
