JOB SUMMARY:
Provides assessment of client needs, treatment planning, counseling, and education services for patients and their families on an outpatient basis coordinating care with the patients, families, and the healthcare team. WORKING CONDITIONS, HAZARDS AND PHYSICAL EFFORT:
Works standard work week Monday through Friday with ability to adjust schedule to meet deadlines or in case of emergencies. Performance of this position in an office environment generally does not involve exposure to blood, body fluids, or tissue. Functions efficiently under moderate to significant levels of stress specific to communication with patients, patient families, community professionals, administrative leadership and clinical management staff. Demonstrates a high degree of independent judgement and initiative to make professional treatment and care decisions. Maintains the ability to independently move or position 25 lbs.
CONTACTS WITH OTHERS:
Maintains routine communication patients and families, clinic staff and providers of the Behavioral Health Clinic including nursing staff, physicians, office managers, department directors, community healthcare providers and referral agencies.
EQUIPMENT USED/SPECIAL SKILLS REQUIRED:
Demonstrates computer skills and competency for use of the Google platform, i.e., Google Docs, Google Sheets, G-Mail, Publisher, and Power Point, computer based reporting systems, Meditech and other system software or programs as required. Demonstrates skills in the development of and implementation of performance and quality improvement standards to contribute to the growth and quality of services at the clinic and throughout the organization. Possesses the professional and interpersonal skills necessary to plan and coordinate a social service program and to work effectively with other disciplines. Demonstrates knowledge of community, social, health and welfare programs.
MINIMUM QUALIFICATIONS OR EQUIVALENTS:
Education: Master's Degree from an accredited school of Social Work.
Qualified for employment by an institution or organization in which the certified social worker has no direct or indirect interest other than employment as a CSW and, as appropriate, meets the requirements to engage in the practice of clinical social work by contracting with a licensed clinical social worker who shall assume responsibility for and supervise the certified social worker’s practice for a minimum of two and one half (2.5) years. Experience: Two (2) years of experience as Social Worker and/or Discharge Planner in Psychiatric clinical setting.
LICENSES, CERTIFICATIONS, AND/OR REGISTRATIONS:
Licensed as a “Certified Social Worker” (KRS 335.080(1)(a)(b)(c)(d)(e)(f)(g).
Non-Violent Crisis Intervention and Prevention (“CPI”) certification within three (3) months of hire.
Crisis Prevention Intervention (“CPI”) annual training.
