Cultural & Community Liaison, Cancer Service Line

This position's priority is to create an environment that is conducive to developing lifelong relationships with members of the community, to increase access to health care services for members of the community and to help our current patients achieve best health outcomes. This person will assist patients who are limited/non-English speaking and help patients navigate the facility, provide linguistically competent and culturally sensitive patient education, and community outreach/screenings as well as assist in identifying, addressing and tracking specific health needs of the community. This position also works on special projects and assignments as requested by the 1-up. This role will support the Cancer Service Line.

Major Responsibilities:

Community Health    Assist in identifying, assessing and tracking specific health needs for the community    Provide linguistically and culturally sensitive patient education for the community and development of community health lecture series at least quarterly    Identify, compile and report to 1-up specific health statistics for the Community in assisting in developing Community Health Needs Assessment.    Work across service lines and departments to coordinate community outreach and health fair programs that include breast cancer screening, colon cancer screening, etc    Participate and engage with community partners and organizations through community coalitions and councils.Community Relations    Attend Community Events and represents site and services available    Develop strong relationships with key community entities related to the identified population.    Promote the cultural program at designated site at available forums in the community.    Respond to community inquiries around the cultural program at designated site.    Promote an environment that meets the unexpressed needs of all those in the facility. The Liaison must have a strong dedication to providing all patients and family members the highest level of customer service. When patients and family members are not satisfied, this person is responsible for utilizing service recovery standards.    Serve as a partner with Business Development and community Chambers of Commerce.Physician Relations/Recruitment    Utilized hospital/systems issue resolution process and ensure personal role as liaison for information between physicians, hospital staff and families when necessary.    Build relationships with physicians though one to one meetings and continued communications to further engage physicians with the hospital.    Present to current referring physicians and potential physicians on referral services, new and current service offerings.    Identify and communicate with Medical Staff Office any local physicians who would benefit through medical staff membership at designated site.    Assist newly credentialed physicians as they come on by providing opportunities.Cultural Competence    Inform clinical support services of any special patient's environmental needs from a cultural perspective.    Create an environment that is focused on service excellence, customer satisfaction, and cultural competence.    Work with physicians and staff at hospital and Center for Advanced Care to create an atmosphere that is sensitive to the cultural needs of the patient and family members.    Inform staff of any cultural sensitivities around the patient's care plan and this include altering care plans to the patient's needs and documenting the reasons for any deviations.Patient Navigation    As needed, translate for patients and their families. This may include explaining a procedure, explaining medications, helping to schedule follow-up appointments, etc. This will require traveling through the Center for Advanced Care and the hospital, communicating with clinical staff regarding the care plan for the patient, and working with physicians.    Assess specific needs of Education materials and make appropriate use of hospital resources to purchase language education materials to develop translations of materials currently in use.    Be sensitive to the functional literacy level of patients and family members. Modifications in conversation should be made for those who are of a lower functional literacy.Other duties as assigned.


Education/Experience Required:

Bachelor’s Degree required with some clinical focus. Ability to demonstrate a superior understanding of the cultural competencies and a strong desire to embody the cultures principles into a large organization At least 3-5 years experience translating clinical information for patients

Experience working with patients and communicating with family member Familiarity with hospital setting Dedication to providing superior customer serviceIf position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case-by-case basis.
