The role you’ll contribute:

Under the direction of the Director of Quality Management, the Cardiovascular Clinical Outcomes Coordinator (CCOC) is responsible for facilitating an evidence based, multidisciplinary, multi-specialty approach to Chest Pain (CP) and Cardiac Cath Lab (CCL) patient care throughout the care continuum. The CCOC is responsible for the effective implementation, monitoring, effectiveness, and maintenance of the ACC Accreditation Programs, to include acquisition and retention of certifications for CP, CLL and HF (HF in collaboration with the HF Coordinator) The CCOC collaborates with physicians from multi-specialties in developing clinical best-practice, coordinating patient-flow processes to treat the Acute Coronary Syndrome (ACS) patient and guide best practices accurately and effectively for the cardiology Cath lab patient. The CCO coordinator will serve as an additional resource for orientation and education of ACC principles to hospital and medical staff as well as patients. The CCOC is responsible for organizing team meetings, abstracting, and compiling clinical data for presentations, completing concurrent and retrospective review of CP and CCL patient records, and following up on team recommendations. The CCOC provides clinical expertise and support to other nursing departments in respect of those patients that present with ACS symptoms and cardiac Cath lab procedural patients. The CCOC monitors and assures that patient care is in compliance with ACC required clinical NCDR registries. The CCOC will additionally be mutually responsible with the HF Coordinator for the above requirements for the ACC HF program.

The value you’ll bring to the team:

Responsible for the implementation, monitoring, effectiveness, and maintenance of the ACC Accreditation Programs, to include acquisition and retention of certifications for CP, CLL and HF (HF in collaboration with the HF Coordinator)

Reviews, abstracts, and evaluates CP, HF, and CCL concurrent and retrospective patient records for compliance with evidence-based standards of care and initiates follow up communication with the multidisciplinary care team members to discuss, confirm, or make suggestions regarding patient management and evidence-based standards. Additionally, performs walking rounds to monitor for compliance with program requirements.

Effectively interprets ACC (American College of Cardiology) Chest Pain Center and Cardiac Cath Lab accreditation standards and develops policies, procedures, and protocols to assure compliance for both programs/accreditations; submits recommendations to medical director, emergency medical director and director of cardiology prior to implementation. Reviews and, if needed, updates these policies/protocols on an annual basis.

Monitors for understanding of applicable standards, policies and procedures as evidenced by performance management reports, incident follow-ups, staff meeting minutes and/or direct observation, as applicable.

Plans, implements, and evaluates the CP and CCL program’s Quality Assessment and Performance Improvement plan to meet the needs of accreditation and certification programs and achieve continual improvement of processes.

In collaboration with the Heart Failure Coordinator, oversees the daily operation of the Heart Failure Program and participates in the planning, implementation, and evaluation of the Heart Failure program’s Quality Assessment and Performance Improvement plan to meet the needs of accreditation and certification programs and achieve continual improvement of processes.

Qualifications

The expertise and experiences you’ll need to succeed:

Bachelor’s degree in nursing upon hire or be actively enrolled in a BSN program within 6 months of hire date

3 years of nursing experience in an acute care setting upon hire

Current active and clear FL Registered Nurse license upon hire

American Heart Association BLS Instructor certification within 9 months of hire date, then renewed every two years as required

American Heart Association ACLS Instructor certification within 9 months of hire date, then renewed every two years as required
