GENERAL SUMMARY The Day Habilitation Case Manager plays a pivotal role in the program's daily operations, ensuring the success and well-being of the individuals served. This position involves developing and managing personalized program service plans and conducting semi-annual reviews in alignment with individual goals and regulatory requirements. The Case Manager collaborates closely with staff and individuals to create, implement, and track meaningful goals and objectives, fostering growth and achievement for all participants. ESSENTIAL JOB FUNCTIONS* + Writes and oversees program service plans and semi-annual reviews for each individual within regulatory requirements. + In conjunction with the interdisciplinary team (IDT), create goals and objectives for all new individuals within 90 days of admission that cover habilitation, medical, social, occupational, and psychological needs. Designs service plan based on Service Needs Assessment (SNA) and defines specific goals based on SNA recommendations. + Communicates with providers, individual’s family, individuals, staff, individuals, and the Department of Developmental Services (DDS), as required. + Prepares semi-annual reviews by summarizing daily and monthly documentation provided by Day Habilitation Services Managers. + Defines behaviorally stated and measurable objectives that outline steps to be taken to reach the goals outlined in the Provider Support Strategies (PSS). + Assists the IDT in planning the frequency and duration for which the services under the service plan will be provided. + Identifies which integrated activities and therapies to include to successfully implement measurable goals within established timelines. + Assists the IDT in planning program activities to successfully implement individual goals. + Adheres to all funding source regulations and Agency policies and procedures. + Keeps management abreast of planned activities, as well as any potential individual issues or needs which are not being met. + Know and be able to use Excel, Microsoft, and other technology programs to create documentation, formats, and templates. + Establishes and maintains professional relationships with community contacts, colleagues, management, and funding sources. + Attends all yearly ISP meetings for individuals that are attending the program in-person coordinated by DDS. Coordinate with DDS, individual’s family, providers, and guardians on scheduling following ISPs. + Attends all IDT, Case Manager & Program Manager, and staff meetings, trainings, and development opportunities, as needed. + Serves as mandated reported for suspected cases of abuse and/or neglect to the Disabled Persons Protection Commission (DPPC). + Be able to maintain, organize, and audit individual charts. With the support of the Day Habilitation Director and Manager due to mandatory audit of all individual charts. + Maintain the required annual documents for individuals within the required charts. + Maintain each individual's emergency contact information in ICentrix. Provide updated Emergency Fact Sheets to all of the necessary parties. + Coordinate with transportation regarding temporary or permanent residential address changes for each individual. Submit PT-1 (transportation request to MassHealth) for each individual when an address change has occurred. Renew PT-1 for individuals to be transported to the program within the required timeline. OTHER DUTIES AND RESPONSIBILITIES + Provides physical assistance to individuals, as needed, in the event of an emergency, behavioral incidents, or staffing shortages, including, but not limited to, assistance with eating and assisting with group activities. + Performs additional duties as assigned. PREPARATION, KNOWLEDGE, SKILLS & ABILITIES + Bachelor’s degree or a minimum of 5 years case management experience. + Knowledge of funding sources and agencies. + Attention to detail. SUPERVISORY RESPONSIBILITY + No supervisory responsibility. WORKING CONDITIONS/PHYSICAL DEMANDS + Normal working conditions. + Frequent standing, walking, bending, and twisting. + Ability to lift at least 50 pounds. + Ability to physically assist and transfer individuals.
