Day Habilitation Instructor

 

• Provide assessment, training and assistance to individuals with developmental disabilities enrolled in HCBS Waiver that are in need of Day Habilitation Services within the community and in the Certified site. • To facilitate an individual’s independence, individualization, inclusion and productivity • Compete Day Habilitation Plan every six months and attend annual and semi-annual reviews with the consumer’s support team. • Provide support to a minimum of eight individuals at one time while in the community driving them to different volunteer sites, stores, and other community establishments. • Complete daily checklists and monthly summaries based on the valued outcomes on the Day Habilitation Plan. • Complete communication sheets to IRA’s and family members requesting communication logs. • Provide group activities as required of the rotation schedule. • Ensure all binders with consumer progress notes are up to date and complete. • Administer medications to consumers that are in need during Day Program hours. • Develop clear and habilitative group activities within the site and in the community. • Support and advocate for all consumer’s within the community. • Performs all other duties as assigned.

Qualifications - External

• Minimum High School Diploma • Valid NYS Drivers License • AMAP Certification • SCIP Certification • Insured Vehicle • OPWDD Fingerprint Clearance SCO Family of Services is an Equal Opportunity Employment Employer.
