Hours of operation:  8:30am - 5pm

Responsible for the psychosocial assessment of people living with dementia and their caregivers as a member of the Dementia Education Program team.Responsible for Care Navigation (including initial GUIDE screenings/assessments and ongoing monitoring/support) for patients aligned to the GUIDE program.Assists in the development of the social services component of the plan of careAssisting individuals with dementia and their caregivers/families within both programs as required by performing the following duties.

 

Qualifications

 

Education/Regulatory Requirements:A Master’s Degree in Social WorkMust have a Valid Florida drivers License, maintain current auto insurance coverage and have access to a reliable automobile.Skills:Minimum of (1) one year experience as a caseworker with an emphasis on counseling, crisis management, community agency referral and grief support, preferred.Experience in developing collaborative relationships with referral sources and first responders.Ability to function independently and to adjust work hours to meet with program participants and to provide outreach and educational programs to family caregivers, healthcare professionals and first responders.Ability to write routine reports and correspondence.Competence in public speaking and engaging family members, healthcare professionals and first responders.Experience with family caregivers of people with dementia and family dynamics.Expertise and knowledge regarding older adults, dementia, dementia care and support, family and informal caregiver needs and challenging behaviors, preferred.Skills and experience in facilitating small group learning and discussion programs; including support groups.Professional Requirements:Ability to operate a facsimile, copy machines, computers and other common office machines, with working knowledge of MS Word, Outlook, Excel and other software appropriate to positions.Excellent communication skills, both verbal and written.
