DMD or DDS from an accredited U.S. programActive Oklahoma Dental LicenseCPR/BLS CertificationWilling to obtain a Real ID for base accessPass a background (Criminal & Malpractice), urinalysis & applicable testing, shots, physical exam and vaccinations.________________________________________
