The DESE/DDS Program is designed to provide in-home services/wrap-around supports to school-age participants; the goal of these supports and services is to help prevent a more restrictive educational or out-of-home residential placement. The intent of the Program is to increase a family’s ability to support his/her child in the home and community. The Program does this by creating an individualized plan of supports for the participant that promotes skill building, independence, and social integration across multiple settings including the participants home, school, and community. What does a DESE/DDS Program Direct Support Professional do? As an AWC Direct Support Professional, you will be responsible for assisting and supporting an individual to develop and practice the skills necessary to pursue opportunities in the community that interest and benefit them. Your key duties will include: + The implementation of Individual Support Plans for individuals based on assessments, objectives, and goals. + The tracking and reporting of data related to goals and objectives. + Actively advocating for the individual with which you are working. + Assisting individuals with disabilities in accessing community resources that will empower them and enhance their quality of life. + Attendance of mandatory training and meetings as assigned and required. Maintain all required certifications, including CPR and CPI. + Complete other tasks as assigned by your supervisor (tasks may vary depending on whom you + are supporting). Qualifications and Skills: + Must pass background checks and pre-employment screenings including TB test and CORI. + High School Diploma required + Basic computer skills using Microsoft Office, email communication, internet, ADP payroll app + Strong interpersonal and communication skills to effectively collaborate with program participants and fellow staff + Compassionate, resourceful, and eager to learn. + Able to use creative problem-solving skills and work independently when needed. + Physical demands may include frequent standing, pushing, pulling, walking, stooping, lifting and/or twisting + Valid driver's license and reliable personal vehicles meeting state requirements.
