The ideal candidate will be passionate about serving our community and some of our most vulnerable neighbors. The person in this role will provide case management services to children and adults with intellectual and developmental disabilities and their families using person centered practices. This position may be assigned other duties and responsibilities as it relates to the Developmental Disabilities program and services. Developmental Disabilities Division on Hwy 101 in Newport Essential Functions/Major Responsibilities JOB FUNCTIONS Provides mandated case management services to eligible individuals which include but are not limited to: + Development of individual support plans and other required documentation. + Conducting needs and risk assessments to determine service needs. + Authorize and monitor services including residential, in-home, and employment services. + Provide information, referrals and coordination to access needed services. + Facilitate all entry, exit and transfers including home and community-based services, the Oregon State Hospital system, State and local correctional facilities, nursing services, support brokerage services. + Take lead in providing for crisis and protective services intervention. + Conduct or participates in licensing and certification processes for service providers in conjunction with State and County licensing staff. Non-essential functions: + Participate to the extent resources allow in Individualized Education Program (IEP) planning meetings and transition programs. + Participate in assessment of community needs, outreach, and planning activities and events. + Attend DD advisory council meetings as needed. + May be delegated civil commitment activities by the DD Program Manager. + Perform other related duties as assigned. Typical Qualifications Minimum Education Level Bachelors Degree Specifics: In behavioral science, social science or a closely related field. Minimum Experience Level One year experience in a related human services field. Substitution Note: A bachelor’s degree in any field and one year of human services related experience; An associate’s degree in behavioral science, social science, or a closely related field and two years of human services related experience; or three years of human services related experience. Certifications None required Driver’s License Oregon Driver's License required within 30 days of hire. Desirable Qualifications: Bilingual and/or bicultural competencies for Latino population. Knowledge of: + Applicable Oregon Administrative Rules. + The public service system for developmental disabilities. + Person centered practices necessary to effectively work with persons with intellectual and developmental disabilities.
