DIABETES CARE & EDUCATION SPECIALIST

Job Overview:

The Certified Diabetes Care and Education Specialist (CDCES) delivers comprehensive and seamless services that bridge the gap and integrate clinical and self management aspects of diabetes and cardiometabolic care. The CDCES is an integral part of the interprofessional team. Team member will collaborate and provide comprehensive, patient-centered care, and education conducive to behavior change and improved quality of life. The CDCES supports and advocates for people affected by diabetes to help the patient achieve individual goals. 

Job Requirements:

Bachelor's Degree in Nursing or Dietitian/Nutrition or Nursing                                                                                               Registered/Licensed Dietitian or Registered Nurse                                                                                                                               Dietitian must hold current license as well as registration by Commission on Dietetic Registration                                               Registered Nurse must hold current license
2-3 years' experience 
Must have certification as Certified Diabetes Care & Education Specialist (CDCED) by CBDCE OR is eligible to sit for certification examination within 24 months of hire date.


Job Responsibilities:


Provides quality diabetes self-management education and medical nutrition therapy (limited to Registered Dietitians) in individual and group settings (both in-person and virtually) based on assessed needs. Utilizes appropriate teaching techniques that are sensitive to the learning preferences of the person with diabetes. Manages virtual classes including presentation of material, answering questions, and pre and post class communication and documentation.

Completes comprehensive assessments for 1:1 patient visits, including emotional and behavioral health, interprets personal health data, develops an individualized care plan based on the patient’ assessed needs and goals and promotes successful self-management. 
Effectively demonstrates ability to evaluate pertinent labs related to diabetes and associated comorbidities such as cardiometabolic, renal health, etc. Routinely reviews lab results and targets with all patients, to ensure their understanding. Works collaboratively with patients and provider to establish targets and individualized goals.
Actively participates as a member of the multidisciplinary team by identifying resources, interventions and strategies to help patient more confidently navigate key areas impacting their diabetes self-management plan including: healthy eating, exercise/physical activity, glucose monitoring, medications, problem solving, healthy coping and reducing disease related risks

Demonstrates ability to manage a panel of patients including initial assessment, development of care plan with patient. Manages patient panel and updates providers regularly with wins/successes as well as patient barriers and needs.  
Demonstrates ability to utilize technology to assist patient with diabetes care and education, in individual and population health diabetes services.
