Position Title: Diabetes Education Specialist

QUALIFICATIONS:

REQUIRED:

Current Minnesota RN or RD License.

Current Wisconsin RN or RD licensure within two months of hire.

Current CPR certification for health professionals through the American Heart Association or Red Cross required within 6 months of hire and maintained. On-line CPR certification is not acceptable.

Strongly demonstrated clinical knowledge/skills in diabetes care, communication (written and verbal), patient teaching / evaluation, and ability to work independently required.

Board certification: Certified Diabetes Educator preferred or CDE eligible, as defined by the credentialing body. If hired without CDE credential, must obtain within 36 months/3 years of hire.

Bachelor’s degree with two years of experience in diabetes care and education, OR 4 years of experience in diabetes care and education.

Professional association membership in American Diabetes Association or American Association of Diabetes Educators.

Position requires travel between sites.



 

PHYSICAL REQUIREMENTS:

Ability to sit or stand for prolonged periods of time. 

Must possess speech, hearing, sight and tactile skills to perform the functions of this position. 

Manual dexterity is required. 

Must be able to speak, read and write English proficiently.



 

POSITION PURPOSE:

Service Excellence is to be centered on patient care and patient relationships and is the responsibility of all employees. Teamwork is the norm and all employees will be held accountable to work as effective team members.

Provides direct care of individual patients with diabetes and populations. The Diabetes Education Specialist is a Registered Nurse or Registered Dietitian practicing in an expanded role, which requires specialized skills and expertise in diabetes. He/she provides consultation, education, and referral among team members. The Diabetes Education Specialist collaborates with the patient’s primary care provider and Endocrinologists to assist in the clinical care and self-management education of the patient with diabetes.



 

ACCOUNTABILITIES:

Conduct a comprehensive assessment of the patient with diabetes.

Develop, implement, and evaluate patient-centered care plans which include intermediate to complex patient education / counseling, collaborative medical management by protocol and collaborative goal-setting with the patient.

Properly document in EMR to meet program requirements in a timely fashion according to organizational policies.

Complete the required amount of continuing education credits on an annual basis as required for maintenance of program accreditation.

Develop and/or implement patient-centered educational programs in collaboration with appropriate HP staff and appropriate professional bodies.

Participate in or utilize research to continue growing professionally.

Assist care team members to implement and sustain practice changes for proactive diabetes management.

Assist with development and implementation of procedures related to diabetes. 

Coordinate patient care with all HP disciplines, across the health-illness continuum, clinical setting, and geographic boundaries, engaging the appropriate resources to meet patient needs.

Provide education and expert consultation to the primary and specialty care teams. 

Participate in appropriate HP and community committees and activities.

Participate in continuous quality improvement initiatives related to diabetes.

Respect and preserve patient rights and confidentiality.

Provide access to service in a timely manner, which meets patient’s expectations.

Perform all duties as assigned.
