Diabetes Educator, RN
**Job Description/Summary:** Reporting to the Endocrinology Practice Administrator and the Medical Director of Endocrinology, the Diabetes Educator is responsible for providing diabetes management education to patients and their families. The educator provides ongoing education and support to diabetes patients in both an outpatient and inpatient settings. This can include bedside education in the hospital, nurse assessments, clinic visits, phone case management, coordination of CCS annual core team visits, and pump consults, pump and censor starts. This position sees patients in the Oakland, San Ramon and Walnut Creek clinics. **Position Requirements:** **Minimum Education:** Bachelor of Science (BS) degree in Nursing **Minimum Experience:** One year of experience working in a pediatric or adult setting (in-patient or out-patient), required. Experience with insulin pumps. continuous glucose monitors, or other diabetes technology preferred. Experience with nurse assessment and intervension skills as part of a multi-disciplinary team preferred. **License Required:** Licensed in the State of California as a Registered Nurse **Certification Required:** Certified Diabetes Educator (CDE), or be able to obtain within 1-year of hire. California Children's Services (CCS) Panel Non-provider, or able to obtain within 1-year of hire. BLS required. **Knowledge, Skills, and Abilities:** Knowledge of patient education/training practices and theories. Knowledge of formulating education/training goals and objectives consistent with patient needs. Knowledge of instructional skills and learning theory, educational. Ability to develop health education programs and instructing patients/families in managing diabetes. Ability to analyze patient education needs and recommending programs to meet needs and in convey patient education concepts to other staff. Ability to exercise initiative, judgement, and decision-making in meeting plan of care objectives. Ability to work effectively as part of a health care team. Ability to creatively problem-solve. Ability to provide own reliable transportation to and from satellite clinics. **Technical Knowledge:** Microsoft Office Suite (Word, Excel, Outlook). Prior experience with Electronic Medical Records preferred. **Physical Requirements:** Sit: Up to 6 hours/day Stand/Walk: Up to 6 hours Bend/Stoop: Up to 3 hours Reach: Up to 6 hours Rep Use of UE/Grasp: Up to 6 hrs Lift/Push/Pull: 25 lbs, over 25# with assistance or equipment This job requires the ability to hear alarms clients and/or instruction.
