Mercy Primary Care is seeking a full-time (40 hours a week) Diabetes Educator to join our team. Overview: The Diabetes Educator plays a crucial role in providing comprehensive education and support to patients diagnosed with diabetes within the outpatient setting. This position involves collaborating with healthcare teams to develop and implement personalized diabetes management plans, empowering patients to achieve optimal health outcomes and improve their quality of life. Qualifications: Education: Bachelor's degree in Dietetics, Health Education, or related field. Certification: Certified Diabetes Care and Education Specialist (CDCES) credential required. Experience: Clinical experience in diabetes management and education, preferably in a hospital or outpatient setting.
