Part Time/First Shift
Under general supervision, this position the Certified Diabetes Educator supports the Joslin Diabetes Center by providing nutritional assessment, education, and management to patients with chronic conditions who are at risk for disease progression, complications, cognitive, and functional decline. In collaboration with the physician and interdisciplinary team, the Certified Diabetes Educator develops and implements an individualized care plan designed to promote the patient’s understanding and management of their condition, optimize quality of life, and improve clinical outcomes. This role will also develop various nutritional programs (adult and pediatric focused) for the Joslin Diabetes Center.
Minimum Education, Training, and Experience Required
Formal training which will be indicated by a graduation certificate from an accredited RN school of nursing, RN license; prefer previous supervisory experience in a healthcare setting and/or experience in clinic-based nursing.
BCLS certification required.
CDE (Certified Diabetes Educator) required.
Certificate of completion for CPT (Certified Pump Trainer) strongly preferred.
Knowledge of nutrition guidelines in chronic disease management.
Knowledge of medical terminology.
Knowledge of HEDIS or NCQA quality measures
Valid unrestricted driver’s license and ability to drive to multiple locations.
