Diabetes Educator - DIABETES EDUCATION

An experienced heath care professional; (for example Registered Nurse, Occupational Therapist, Social Worker et cetera) Certified Diabetes Educator with skills and knowledge who utilizes the nursing process to provide direct and indirect patient/participant diabetes education. Responsible for program planning, implementation, and evaluation. Educator and resource person to patients and staff. Works independently with little supervision.

Required Qualifications:

Graduation from an accredited healthcare professional program (RN, SW, PT, OT, etc.). Bachelor's Degree healthcare education related program. within 1 year of hire Certified Diabetes Educator (CDE) - National Certification Board for Diabetes Educators. 2 years Diabetic self care management experience.
