Key Responsibilities:
Identifies and assesses patient nutritional needs and creates care plans to address any identified risks. This may include but is not limited to working with critical care patients in the ICU, writing nutrition-related orders. related orders, providing appropriate nutrition education, assessing wound-related nutrition needs and completes timely and thorough documentation in the electronic medical recordAssesses for and diagnoses malnutritionEducates on diet according to disease state, medical condition, and health riskCoordinates discharge plans for patients returning home with nutritional support needs. This includes, but is not limited to, writing orders for home nutrition support equipment, creating and educating patient on nutrition support plans, navigating insurance plans for coverage of home nutrition support, and coordinating needs with the selected home medical supply company of patient’s choiceActs as consultants to rural Essentia Health site providers for high acuity nutrition care concernsLicensure/Certification Qualifications:Certification/Licensure Requirements:

Registered or registry-eligible with the Commission on Dietetic Registration of the American Dietetic Association within state(s) of employment. Must be registered within state of employment within six months of hire or transfer
