What you'll do Under the supervision by the attending physician and the Program Coordinator for Diabetes Management, the Diabetes Nurse Specialist acts as an expert resource in the field of diabetes. The Diabetes Nurse Specialist functions as a critical team member of the Endocrine Division, delivering care to patients in this subspecialty. Contributes to education of family, community and hospital staff as appropriate, and participates in medical and nursing research as indicated. The Diabetes Nurse Specialist works closely with the Program Coordinator on such issues as staffing, quality assurance, and equipment ordering when and if necessary. This individual must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of patients served. This individual must demonstrate the ability to assess data reflective of the patients’ status, identify each patient’s requirements relevant to his or her age specific needs, and provide appropriate care. Provides care to patients primarily in the outpatient setting. Utilizes principles of nursing process in meeting patient age specific needs with regard to safety, hygiene, nutrition, and medication administration while also attending to physiological, spiritual and emotional needs a. Collects patient health data and reviews with physician as appropriate b. Conducts/coordinates outpatient diagnostic tests/treatments according to established protocols c. Identifies expected outcomes individualized to patient 2. Provides appropriate patient and family teaching and support a. Explains treatments and procedures and answers patient questions b. Participates in the development of patient teaching materials c. Advises modifications of self-care within treatment plan and refers to physicians, as necessary d. Receives phone inquiries from patients, identifying issues and concerns and the need for appointment or appropriate intervention 3. Acts as an expert in the field of Diabetes Management, overseeing the clinical orientation of new nursing and support staff and arranging for in-service training as appropriate a. Assists new clinic nurses on specifics of Diabetes Management b. (Certified Educator) Instructs new clinic nurses on specifics of Diabetes Management c. Functions as a Diabetes resource for the Bassett Healthcare Network clinics assigned d. Works to arrange or participate in-services on diabetes management and related topics e. Helps with orientation schedules for new nurses and staff as assigned f. (Certified Educator) Orients new Diabetes Nurse Specialists 4. Working closely with the Program Coordinator, functions as a key member of the Diabetes Education program and the Endocrinology Team a. Set an example in work performance and team building attitudes, concern for patients and support of clinicians by maintaining a positive interpersonal attitude and demeanor, as observed by the program coordinator b. Resolves patient issues as they relate to Diabetes Management operations, forwarding to the Program Coordinator as necessary, and as observed by the Program Coordinator c. Works with Program Coordinator to assure compliance with all outside accrediting and regulatory agency codes and standards, as observed by the Program Coordinator d. Works directly with the Program Coordinator to implement Quality Assurance/Risk Management activities and practices, as observed by the Program Coordinator 5. Assists and/or provides teaching and consultative services to nurses, other health professionals, community health providers, and participates in public education, as time permits a. Assists with educational programs to nurses, students, and other staff members to improve knowledge of the care of diabetes patients b. (Certified Educator) Coordinates and presents educational programs to nurses, students, and other staff members to improve knowledge of the care of diabetes patients c. Participates on educational activities of other departments and community programs d. Participates on committees, reports nursing aspect of patient care, as indicated 6. Assists Program Coordinator with financial accountability including oversight of equipment ordering and maintenance processes and department billing a. Is knowledgeable in correct billing methods, ensuring EMR notes, and billing codes are completed correctly with 90-95% accuracy rate b. Ensures that supplies are ordered in the most cost-effective manner possible, alerting program coordinator of potential issues as they arise 7. Performs similar or related duties as requested or directed a. Performs other duties as requested and observed by the Program Coordinator What we need from you Education: 4 Year/Bachelor's Degree in Nursing or related field within 1 year of hire, required Experience: Minimum 3 years nursing experience, required Licensure/Certifications: Must complete American Associates of Diabetes Educators 1 program within 8 months from date of hire, required Must become Certified Diabetes Educator within 3 years from date of hire, required Current NYS RN License, required Skills: Accountability Advocacy Clinical Skills Communication Interacting with People Manage Programs Reading/Writing/Comprehension Resourcefulness Responsibility Teaching Time Management Physical Requirements: Medium Work - Exerting up to 50 lbs. for force occasionally, and/or up to 20 lbs. of force frequently, and/or up to 10 lbs. of force constantly to move objects. Must be able to pull/push more than 50 pounds and lift objects more than 100 pounds with assistance Must be able to perform moderately difficult tasks such as positioning patient, recording treatments administered Must be able to stand for long periods of time Must be able identify and differentiate colors. Pay Transparency Salary is based on a variety of factors, including, but not limited to, qualifications, experience, education, licenses, specialty, training, and fair market evaluation based on industry standards. Pay Range Minimum: USD $77,981.34/Yr. Pay Range Maximum: USD $116,972.01/Yr.
