Identifies patients at nutrition risk and assesses nutrient requirements according to age and disease specific needs. Develops a Nutrition Diagnosis and implements nutrition care treatment plans for all assigned patients. Continuously evaluates and monitors the effectiveness and outcomes of MNT intervention. Responsibilities: EDUCATION + BS Degree in Nutrition/Dietetics plus internship or other education to meet requirements for registration with the Commission on Dietetic Registration (CDR) + Registration with the CDR + Registered Dietitian or registration eligible. Registration must be obtained within 6 months of employment + Certified Dietitian-Nutritionist (CDN) with New York State to obtained within 1st year of employment + Active membership with the Academy of Nutrition and Dietetics preferred EXPERIENCE + 1 year experience preferred KNOWLEDGE, SKILL AND ABILITY + Must possess or have ability to learn basic computer skills WORKING CONDITIONS + Occasional exposure to one or more mildly unpleasant physical conditions + Travel between Catholic Health System facilities may be required + Occasional evenings may be required for nutrition counseling appointments or classes REQNUMBER: 31792
