ResponsibilitiesImplement the Nutrition element of the Program intervention.
Assist the Program Director with efforts to actively promote the Program to prospective participants and health care professionals as needed.Provide nutrition education, counseling, and coaching to participants about the implementation of the Program's nutrition guidelines.Deliver nutrition lectures/sessions and cooking demos/classes according to Program guidelines.Conduct a nutritional assessment for nutritional adequacy on each participant.Communicate clinical issues to the nurse case manager and medical director.Participate in the development of the interdisciplinary treatment plan to ensure the quality and clinical effectiveness of the program, and the safety of each participant.Implement the Nutrition element of the Program intervention. Assist the Program Coordinator with efforts to actively promote the Program to prospective participants and health care professionals as needed.Provide nutrition information for the community that may include public speaking, news releases, and other forms of publication.Conduct a nutritional assessment for nutritional adequacy on each participant. Collaboratively sets goals with participants in order to achieve adherence to Program guidelines. Document's the participant's response to education, counseling and coaching.Develop meal plans, taking both cost and clients' preferences into account.Evaluate the effects of meal plans and change the plans as needed.Promote better health by speaking to groups about diet, nutrition, and the relationship between good eating habits and preventing or managing specific diseases.Keep up with or contribute to the latest food and nutritional science research.Write reports to document patients' progress.RequirementsLicensure / Certification / Registration:
Registered Dietitian/Nutritionist credentialed from the Commission on Dietetic Registration obtainedprior to hire date or job transfer date required.
Education:
High School diploma equivalency with 2 years of cumulative experience OR Associate'sdegree/Technical degree OR 4 years of applicable cumulative job specific experience requiredAdditional Preferences
