Dir Quality and Clinical Transformation

Summary:

The Director of Quality and Clinical Transformation plays a pivotal role in BHMG Primary Care�s performance and is responsible for identifying and closing gaps in care achieving pay-for-performance and incentive program requirements and meeting quality measure targets for every payer and program. This role is also crucial for Clinical Operations and Access throughout primary care practices and overseeing the Behavioral and Community Health program.

Responsibilities:

Develops implements and evaluates quality programs to ensure that patient care is consistent with the organizational mission to improve the health of our patients improve the care we deliver and lower the cost of care.

Develops implements monitors and directs comprehensive patient-driven quality programs that support BHMG Primary Care�s business objectives and result in improved quality increased patient satisfaction lower medical costs and enhanced levels of evidenced-based medicine performance.

Lead manage and achieve accurate and timely reporting required to meet advanced payment programs and clinical and financial objectives. Organize and prioritize stakeholders' data needs to ensure success in achieving operational goals.

Leads and manages the quality program and coordinates with the Clinical Quality Manager the projects assigned to Quality Specialists and other quality team members.

Lead and manage expansion of Behavioral and Community Health team operations.

Oversee and lead daily clinical operations to ensure efficient high-quality care delivery.

Ensure the successful performance of all Clinical Operations initiatives and track key performance indicators.

Identify and develop workflows to address quality measure performance deficiencies related to racial sociodemographic or gender differences.

Synthesizes and communicates information efficiently and constructively to the management team clinicians and practice managers so that all stakeholders in clinical quality can make informed data-driven decisions to improve performance.

Ensures the accuracy integrity and timeliness of our data reporting to all stakeholders.

Collaborates with the Finance and Data Analytics departments to ensure adherence to data needs related to risk contracts and quality performance programs.

Promotes the concepts and practice of quality improvement and identifies systemic opportunities to optimize the clinical performance of Coastal practices on quality measures or initiatives.

Works collaboratively with internal stakeholders practices and consultants on essential data collection reporting and analysis.

Works collaboratively with providers staff and executive management to design and implement workflows that maximize quality performance and minimize burden on clinicians.

Develop strong relationships with clinicians practice managers and support staff to ensure the achievement of quality initiatives.

Educates staff regarding quality measures performance standards and proper documentation including creating and delivering formal presentations on quality and incentive programs.

Communicates and interprets requirements of Advanced Payment Programs articulates data needs and evaluates the impact on primary care. Communicates quality plans timelines and deliverables to practices and Brown Health Medical Group Primary Care clinical program directors.

Perform and/or review the monitoring and auditing activities related to contract requirements and coordinate remediation as needed.

Participates in committees and workgroups as assigned.

Performs other job-related duties as assigned.

EDUCATION:

Masters or advanced degree required

EXPERIENCE:

Minimum of 5 years experience exhibiting advanced performance in healthcare data management clinical workflow quality measure reporting

Three or more years of experience implementing clinical quality improvement initiatives is preferred.

SUPERVISORY RESPONSIBILITY:

Up to 5 direct FTEs.
