Direct Support Professional
Description of Responsibilities
Working in the home and community environment responsible for the delivery of quality one on one services to Intellectual and Developmentally Delayed individuals and implementing their goals as outlined in the Person Centered Plan. Care may also involve activities of daily living and communication with other members of the individuals’ multi-disciplinary team. Completion of all documentation in accordance with state and program regulations, Aveanna Healthcare policies and procedures, and any applicable accreditation standards.
Driver’s License and Personal vehicle with up to date insurance and registration required.
No experience necessary, we will provide and pay for your training!
Minimum Qualifications
+ Experience with ID/DD individuals preferred, but not required
+ Proof of eligibility to work in the United States.
+ High school Diploma or GED.
+ Active Drivers License and current automobile insurance.

At Aveanna, a DSP's responsibilities include helping in our clients' Everyday Lives by nurturing opportunities and supporting the individuals in their home and community.
Responsibilities
+ Implement goals from ISP with consumer 1:1 services including but not limited to in home and community supports, companion services, and respite services.
+ Instruct and reinforce target goals and outcomes according to ISP.
+ Complete written documentation of services(s) provided upon completion of visit.
+ Maintain appropriate professional conduct at all times.
+ Assist in the development of consumer ISP and goal revisions as directed by team.
+ Provides care in accordance with written instructions from professional staff.
+ Reports immediately to supervisor any changes or concerns in the patient’s condition or any problems in assigned tasks.
+ Is dependable and on time for assignments.

+ Good organizational and communication skills.
