Direct Support Professional II - Service Coordinator- ADH Type Regular Full-Time Job ID 31131 Schedule Monday-Friday 7:30am-3:30pm Location : Address 81 Hope Avenue Overview We’re looking for a compassionate and hands-on individual to join our team in a role focused on coordinating and providing support services for participants. This position involves assessing, planning, implementing, and monitoring the services required to meet participants' needs, as well as referring to outside resources when appropriate. You will document attendance and assist with comprehensive assessments and care plans. Additionally, you’ll play a key role in developing and facilitating relevant group activities for participants. Flexibility is essential, as you’ll also provide direct support to meet daily operational needs, including assisting individuals with personal care activities. Responsibilities

Inform participants of and refer them to available community services including specialized counseling.

Participate in the completion of comprehensive assessments and plans of care, complete service coordination notes minmally every 90 days per participant.Provide as appropriate coordination of the medically-related physical, mental and behavioral service needs of participants.To act as Case Manager for individuals including case record maintenance, as assigned.To work directly with program participants and their significant team members in the development and follow up of Individual Support Plans (ISP), including attendance at case conferences.Provide assistance as needed for program participants’ personal care needs including dining, toileting and transfers.To develop and implement behavior management interventions for individuals, as warranted and in conjunction with the clinical team.To participate in program and/or agency committees as assigned.To design and implement instructional units on both an individual and group basis to meet each individual’s needs and interests.To participate in activities as a member of the Interdisciplinary Team as assigned including overisght of daily participant attendance and transportation services tracking that is compiled into the pre-billing form.To assist the Program Director in the screening of prospective participants, as assigned.Be familiar with, and assist in the implementation of, the Seven Hills Foundation Policies & Procedures, the site’s Program Specific Policies & Procedures and the regulations governing the program.To foster the growth of individuals by teaching and offering opportunities for choice, initiation and risk taking.To treat individuals with respect and dignity and in an age and culturally appropriate manner. Qualifications

Minimum of a bachelor's degree in human services or a related degree and at least one year of relevant and recent experience working with adults in a professional capacity is required. Experience with adults with developmental disabilities is preferred. Licensed social worker preferred. Valid driver's license required.
