Responsibilities include offering hands-on assistance in daily living skills, implementing health and behavior support plans, facilitating community activities, and documenting progress. The role involves planning recreational and vocational opportunities, assisting with personal care and household tasks, providing transportation, and ensuring a safe, positive environment. Additional duties may include participating in team meetings, developing care plans, maintaining facility upkeep, and adhering to privacy regulations. The position also requires ongoing learning and communication with supervisors and other team members to ensure high-quality support.
