Director, Clinical Transformation

Description:POSITION PURPOSE

Provides leadership, consultation, and coordination for the transformation of clinical processes in order to achieve patient-centered care across the system.  This transformed patient-centered care will increase the organization’s ability to provide safe, effective, efficient, and best practice clinical care across the continuum of care.

ESSENTIAL FUNCTIONS

Knows, understands, incorporates, and demonstrates the Trinity Health Mission, Vision, and Values in leadership behaviors, practices, and decisions.

Provides Clinical Leadership and works collaboratively with Information and Technology Services (ITS), Human Resources, Regional Health Ministries (RHMs), Physician Network Offices (PNO), Clinical Integration Network (CIN), Insurance and Risk Management, Supply Chain, and others to improve care delivery across the continuum of care.

Demonstrates leadership, collaboration, critical thinking, and teamwork when making decisions and resolving issues.  Leads interdisciplinary functional collaborative through the 4 phases of a collaborative clinical improvement cycle (pre-design, design, implementation and sustain/improve).

Leads the collaborative support team members including change leadership, process excellence, project management, clinical analytics, and external experts.  Assures that collaborative financial objectives (savings/costs) are achieved.

Holds RHM collaborative teams accountable for implementation of collaborative processes and achievement of collaborative goals.

Facilitates visionary, out-of-the-box thinking throughout current and future state workflow processes.  Develops list of important decision points to achieve best practice, evidence-based future state flow for each key situation.

Utilizes Lean, Six Sigma, Change Leadership, PDCA and other quality improvement tools to advance collaborative work in a manner consistent with the Unified Clinical Organization (UCO).  Demonstrates the attributes of a servant leader.

Understands and articulates the “connections” and links between and among RHM and system office staff working to advance clinical transformation.

Works within the UCO decision-making structure and reports activities to promote communication and accountability.  Provides ongoing communication to the UCO Leadership Team, Operations Team, UCO Sponsorship and RHM leadership.

Understands, supports, and participates in the integrated work of the UCO department which includes:

             Physician relationships and collaboration regarding care decisions

             Development and implementation of a nursing practice

             Physician office decisions and support

             Quality and safety measures and action plans

             Regulatory issues

             Pharmacy and medication safety

Models teamwork within the department.  Utilizes support staff appropriately and willingly learns to use tools and technology to manage projects and documents (project mapping, e-project, Visio, and Power Chart). 

Assists in the handling of multiple projects/assignments simultaneously and adapts to frequent changes in priorities.

Collaborates in sharing information externally through publishing, conference presentations and others in accordance with the Trinity Health policy.

Maintains a working knowledge of applicable Federal, State and local laws/regulations; the Trinity Health Integrity and Compliance Program and Code of Conduct; as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical and professional behavior.

MINIMUM QUALIFICATIONS

Must possess an in-depth, advanced clinical base of knowledge as normally obtained through an Advanced Clinical Degree or public health degree and a minimum of five (5) years of related clinical experience.  Strong management and leadership experience working in the capacity of a Nursing Director, Chief Nursing Officer, seasoned Nurse Manager, or Clinical Practice Leader is highly desirable.

Must possess a minimum of two (2) years of experience in managing performance improvement activities, including analyzing clinical workflow processes.

Familiarity with clinical information systems, clinical software, and other computer applications.  Prior experience and knowledge of electronic health record functionality is desired.

Must possess excellent verbal and written communication skills, presentation skills, advanced analytical skills, be well organized, and maintain a highly professional manner and appearance.

Must be able to operate effectively in a collaborative, shared leadership environment.

Must possess a personal presence which is characterized by a sense of honesty, integrity and caring with the ability to inspire and motivate others to promote the philosophy, mission, vision, goals and values of Trinity Health.

PHYSICAL AND MENTAL REQUIREMENTS AND WORKING CONDITIONS

Must be able to adapt to frequently changing work priorities, and be able to prioritize and balance the requirements of working with the System Office and Regional Health Ministries (RHMs).

Must be able to set and organize own work priorities, and adapt to them as they change.

Must be able to travel to the various Trinity Health sites, planned conferences as needed (Minimum 30%, up to 80%).
