Physician Affiliate Group of New York (PAGNY) is adding a Director of Primary Care to the Adult Primary Care Practice at Jacobi Medical Center, located in the Bronx. The practice cares for a large and diverse population of medically complex ambulatory patients. NYC Health + Hospitals/Jacobi Medical Center is affiliated with NYC Health + Hospitals, the country’s largest public health system. The practice is in our state-of-the-art ambulatory pavilion, adjacent to the inpatient facility in the northeast Bronx. We are a designated Level III Patient-Centered Medical Home using clinical innovation to improve patient outcomes. The hospital maintains an academic affiliation with Albert Einstein Medical College.
