Job Summary
Manages, through an integrated process, the area of Care Coordination, Discharge Planning, and Social Services for Inter-Community and Foothill Presbyterian Hospitals. Serves as liaison between acute and post-acute care. Designs, develops, implements, evaluates and revises the delivery of the service to ensure optimum patient outcomes with appropriate utilization of services. Serves as liaison with community resources to promote networking. Promotes organizational interdisciplinary collaboration and establishes close links with the Departments of Finance, Managed Care, and Health Information.
Job RequirementsMinimum Education Requirement:
Bachelor’s degree in nursing or in a related field preferred. Ability and knowledge in program development. Effective written, verbal and time management skills. Ability to supervise and motivate employees, establish relations and function within a team. Knowledge of the delivery of Social Services. Knowledge of medical information systems
Minimum Experience Requirement:Two (2) years managerial experience in care coordination, discharge planning, utilization management and/or other related area. Excellent customer service required.
Minimum License Requirement:California RN license. BLS required.
