ResponsibilitiesResponsibilities:
Formulate and implement operational strategies and initiatives in the case management program.Direct and evaluate all services provided, including care plan development, service level determination, and complaint management to achieve performance and quality control objectives.Provide specialized expertise, ensure resource alignment with system-wide initiatives, and update appropriate parties on case management strategies, initiatives, outcomes and issuesSuccessfully partner with physicians and others to ensure delivery of evidence-based care and reduction of avoidable clinicalvariation in care.Requirements
Licensure / Certification / Registration:
Required Credential(s):One or more of the following:
Case Manager credentialed from the Commission for Case Manager Certification (CCMC) obtained prior to hire date or job transfer date.Counselor obtained prior to hire date or job transfer date.Registered Nurse credentialed from the Texas Board of Nursing obtained prior to hire date or job transfer date.Social Worker credentialed from the Texas State Board of Social Worker Examiners.Respiratory Care Practitioner obtained prior to hire date or job transfer date.Respiratory Therapist credentialed from the National Board for Respiratory Care (NBRC) obtained
prior to hire date or job transfer date.Social Worker obtained prior to hire date or job transfer date.
Education:
High school diploma/GED with 2 years of experience, or Associate's degree, or Bachelor's degree required. Master's degree preferred.Work Experience:
5 years of experience required.10 years of experience preferred.2 years of leadership or management experience required.5 years of leadership or management experience preferred.Additional PreferencesCase Management experience required
Leadership experience required
Human Resources experience preferred
