Job Overview:
This role has responsibility and accountability for coordinating, facilitating, and monitoring patient care and support that is provided to hospice patients and their families in all locations where HOC provides services. This includes patient care outcomes, service excellence, human resource issues, fiscal management, quality improvement, compliance, and communication. Works in collaboration with HOC Senior Leadership and others on strategic operational issues to maximize success of the entire HOC organization. Promotes development of innovative interdisciplinary programs designed to create the best possible personalized and meaningful advanced illness and end of life experiences for those who need care in our community.
Job Requirements:
Master's Degree in Nursing; or related degree and incumbents with degree obtained by 2014
CHPN (Certified Hospice and Palliative Nurse)
Registered Nurse
Knowledge of legal and regulatory requirements of hospice and palliative care
Continuous improvement methodology in a healthcare environment
Healthcare economics and Healthcare policy
Active membership in a professional organization
5-7 years experience Management Nursing
4-5 years experience Management Nursing Hospice and/or Home Health Experience
