This individual will oversee the development, implementation, and continuous improvement of clinical strategies aimed at enhancing patient outcomes, optimizing care delivery, and ensuring operational efficiency. The Director will work closely with clinical teams, senior leadership, and external stakeholders to maintain the highest standards of care while ensuring compliance with state and federal regulations.
Ensures that all activities performed align with the vision of Elara Caring’s board of directors, executive team, and the leadership of the Personal Care Services team. Strategic Leadership & Planning:Lead the development and execution of strategic clinical initiatives that enhance the quality of care and optimize patient outcomes across all home care services.Work with senior leadership to set clinical priorities, align care delivery models with business objectives, and identify opportunities for efficiencies.Design and implement innovative clinical programs to address emerging needs in the home care industry, including reduction of hospitalizations, VBP, Care Coordination, and patient education.Clinical Program Development & Oversight:Oversee the design, implementation, and evaluation of clinical programs and initiatives in personal care and licensed home care services.Ensure that all clinical initiatives are aligned with industry best practices, regulatory standards, and organizational goals.Develop, implement, and continuously assess care protocols, guidelines, and clinical policies to ensure high-quality, safe, and effective home care services.Quality Improvement & Compliance:Lead quality improvement initiatives aimed at enhancing the clinical outcomes, patient satisfaction, and overall effectiveness of home care services.Monitor clinical performance through data analytics, patient feedback, and other quality measures to identify areas for improvement.Ensure compliance with all New York State and federal regulations, including those set by the New York State Department of Health (DOH) and Centers for Medicare & Medicaid Services (CMS).Collaborate with internal teams to prepare for and respond to regulatory audits and surveys.Clinical Team Collaboration & Support:Work closely with clinical staff, including registered nurses, Licensed Practical Nurses, and other healthcare professionals, to ensure the effective implementation of clinical initiatives and best practices.Provide guidance, training, and support to clinical teams, fostering a culture of continuous learning and professional development.Serve as a resource to clinical staff by offering expert advice on clinical protocols, care delivery, and quality improvement strategies.Collaborate with clinical leadership to ensure consistent, high-quality care is delivered in alignment with organizational goals and regulatory standards.Oversee and ensure the effectiveness of the HHA and PCA training program, ensuring all aides receive proper orientation, ongoing training, and competency evaluations in accordance with regulatory requirements and organizational standards.Collaboration & Stakeholder Engagement:Collaborate with interdisciplinary teams, including case managers, physicians, social workers, and other healthcare professionals, to ensure coordinated and comprehensive patient care.Cultivate and maintain relationships with external stakeholders such as healthcare providers, payers, regulatory bodies, and community organizations.Act as a subject matter expert on clinical trends and innovations in the home care space, representing the organization at relevant meetings and conferences.Data-Driven Decision Making & Reporting:Utilize data analytics to monitor clinical outcomes, track performance metrics, and identify areas for improvement.Prepare and present reports on clinical performance, key initiatives, and outcomes to senior leadership and stakeholders.Use data insights to drive continuous improvement efforts and support evidence-based clinical decision-making.Promotes Elara Caring’s philosophy, mission statement and administrative policies to ensure quality of care.Maintains patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule.Performs other duties/projects as assigned. What is Required?EDUCATION: Bachelor’s degree in Nursing (BSN) required; Master’s degree in Nursing (MSN), Healthcare Administration, or a related field preferred.EXPERIENCE:8-10 years of experience driving clinical and operational efficiencies in a licensed home care setting, with a proven track record of utilizing technology to streamline processes and improve outcomes.8-10 years of experience in implementing and managing Value-Based Purchasing (VBP) programs within a licensed home care environment, demonstrating a successful history of program design, execution, and performance optimization.CERTIFICATION/LICENSURE: Current, active Registered Nurse (RN) license in New York State.SKILLS: Strong background in clinical care delivery and home health care models, with a focus on leveraging technology for operational improvements.Proven ability to design and implement VBP programs that align clinical practices with financial and quality performance targets.Expertise in using data analytics, healthcare technologies, and electronic health records (EHR) systems to optimize clinical workflows and enhance patient care.Excellent leadership, communication, and interpersonal skills, with the ability to drive change and foster collaboration across multidisciplinary teams.Proficiency in Microsoft Office Suite and electronic health record (EHR) systems.TRANSPORTATION: Must have a dependable vehicle, a valid driver’s license, and current auto insurance in accordance with the laws of the State.
