We are seeking an innovative and dynamic Director of Business Development to lead and expand our Home Health services. This role is pivotal in driving growth, developing strategic partnerships, and enhancing our presence in the market.

Key Responsibilities:

-Coordinate services for patients discharged from hospitals and other referral sources

-Obtain valid Medicare and other referrals as outlined in offer letter

-Identify patients PCP and obtain authorization for home health services

-Obtain doctor signatures for various home care orders and document accordingly

-Assist in development of patient care plan and coordinate home care needs

-Other duties as assigned by supervisor

 

Schedule:

Day shift Monday to Friday

 

Experience:

Home health: 1 year (Preferred)

License/Certification:

Driver's License (Required)
