General Summary
Establishes, administers, and directs the operational aspects of the Crisis Bridge Program Team, a clinical team which assumes responsibility for directly providing needed treatment, rehabilitation, and support services to identified patients with severe and persistent mental illnesses; supervises and evaluates staff performance to ensure service excellence and courteous, helpful, and respectful services to program consumers; and functions as a practicing clinician on the team as well as a liaison with funding, regulatory, community, and insurance agencies, while maintaining accurate reports.
+ Scheduling staff work hours to assure appropriate coverage for day, evening, weekend, and holiday shifts and on-call hours.
+ Lead the daily organizational staff meetings and treatment planning meetings.
+ Continuously evaluate the status of patients and do appropriate planning and coordination of treatment activities to ensure immediate attention to their needs.
+ Direct and coordinate the patient intake process and treatment, rehabilitation, and support services of the program in coordination with the LMHP.
+ Coordinate the intake process which involves scheduling the intake interview, developing, and coordinating the initial assessment and initial treatment plan, assigning the most appropriate staff to the individual treatment team, and providing clinical supervision of the development of the comprehensive assessment and treatment plan.
+ Direct and coordinate, for each patient, the client-centered comprehensive assessment of psychiatric history, mental status, and diagnosis; physical health and dental health; use of drugs or alcohol; education and employment; social development and functioning; activities of daily living; and family structure and relationships.
+ Participate in staff recruitment, interviewing, hiring, termination, work assignments, and orientation and performance supervision.
+ Develop and implement staff orientation and training schedule.
+ Develop and administer the CBP program budget in consultation with the CFO and Accountant assuring that the program budget runs with established parameters.
+ Supervise medical records management assuring maintenance of the medical record in compliance with agency policy, state regulations, and Medicaid payment requirements.
+ Train staff on medical record requirements.
+ Supervise individual staff for medical records mastery through regular review of patient assessments, treatment plans, and progress notes written by the staff.
+ Develop and facilitate quality improvement activities and reviews of the program in coordination with the Quality Improvement Director and LOC Director.
+ Develop and maintain program policies and procedures and revise as necessary.
+ Initiate and maintain professional and collaborative relationships with human service agencies and informal community resources, such as landlords and employers.
+ Participate in and conduct parts of the patient-centered comprehensive assessment.
+ Provide care coordination, including developing, writing, implementing, evaluating, and revising treatment goals, providing individual supportive therapy and symptom management, and supporting patients' families.
+ Provide direct clinical services to patients on an individual or group basis in the office or community settings to teach behavioral symptom-management techniques to assist patients to adapt to and cope with internal and external stresses.
+ Provide practical help and side-by-side coaching, problem-solving, training, and supervision to help patients obtain the necessities of daily living.
+ Develops Performance Improvement tools and processes. Collects and uses data to improve the services provided.
+ Facilitates involvement of patient, family, and interagency team members in treatment and discharge planning.
+ Orients patients, families, and community/school personnel to mental health treatment methodologies, procedures, and services.
Common Expectations:
+ Completes yearly annual safety education modules in compliance with WellSpan Philhaven policy.
+ Contribute to a safe work environment through knowledge and practice of safe behavioral management practices, fire safety, physical plant safety, safe emergency practices, and universal precautions for body fluids.
+ Maintains valid driver's license.
+ Complies with all applicable WellSpan Philhaven standards of documentation.
+ Participates in the on-call system according to WellSpan Philhaven CBP policy.
+ Attends internal trainings as scheduled.
+ Attends external training or continuing education offerings.
+ Assures compliance with WellSpan Health's mission and values.
+ Participates in administrative staff meetings and attends other meetings as assigned.
Minimum Education:
+ Master's degree in counseling, psychology or human service-related field required
Work Experience:
+ 3 years in a professional mental/behavioral health setting required
+ Experience in clinical social work, psychology, or a closely related field required
Licenses:
+ Licensed Clinical Social Worker Upon Hire Preferred or
+ Licensed Professional Counselor Upon Hire Preferred or
+ Licensed Marriage and Family Therapist Upon Hire Preferred
Driver's License Statement:
Must possess a valid driver's license in the current state of residence. Driving record must meet requirements established by WellSpan Risk Management.
Courses and Training:
+ Workplace Violence De-escalation (WVD) Training Upon Hire Required
Knowledge, Skills, and Abilities:
+ Professional level of knowledge of patient care assessment, treatment planning, and behavioral interventions.
+ Excellent human relations and oral/written communication skills.
+ Analytical abilities.
+ Ability to work under stress.
+ Knowledge of and ability in the care and handling of clients in at least the adult and older age group.
