Requirements
Assists in the overall administration and management of the daily clinical and operational functions of the Calvary Hospice. To coordinate, direct, supervise and evaluate the coordination of patient care services consistent with the philosophy, polices and standards of the Agency. The Director of Patient Services is responsible for the clinical operations of the Hospice and Home Care. Provides leadership and coordinates the clinical activities of the interdisciplinary team. Responsible for the provision of skilled IDT assessments and the implementation of coordinated clinical care by all disciplines of the Hospice and Home Care teams.
Salary: $160,000 - $180,000/Annually
Qualifications:
+ Current N.Y.S. Registered Nurse License
+ Minimum of six (6) years of professional nursing service, which should include at least two (2) years in Community setting. Minimum of two (2) years supervisory experience in community setting.
+ Current Driver’s License preferred and insurance
+ Ability to communicate effectively and sensitively with other staff members, referral sources, patients and their families and physicians.
+ Exhibits an ability and attitude relative to the spirit of caring and tradition at Calvary Hospital as required by specific job duties and functions.
Education:
+ A Baccalaureate Degree in Nursing from an accredited school is required
+ Master’s Degree in health-related field required.
