Director of Social Services (Full Time) AristaCare at Norwood Terrace is seeking a Director of Social Services to join our team. In this role, you will provide medically-related social services to attain or maintain the highest practicable physical, mental, and psycho-social wellbeing of each patient. + Develop and implement case management services to all residents. + Maintain contact with family (with resident’s permission) to report on changes in health, current goals, discharge planning, and encouragement to participate in care planning. + Maintain progress notes under federal and state guidelines for Long Term Care. + Facilitate the building of relationships between residents and staff by teaching staff how to understand and support residents’ individual needs. Requirements:  + LCSW or CSW is required. Must be a licensed social worker. + BA from an approved Social Work school is required. + Subacute and geriatric experience + Minimum one years’ experience with seniors.
