Discharge Care Navigator (Full Time, Baltimore Washington Campus)

General Summary:

In collaboration with the patients' interdisciplinary treatment team, performs a full range of aftercare and discharge planning services geared to mentally ill patient population.  Maintains comprehensive knowledge base of agencies, organizations, and resources within the referral community for discharge planning and placement.  Communicates with patients based on an understanding of the developmental needs of the population.  Develops and assists in the implementation of discharge and aftercare plans; works with patients and families to facilitate the community transition and assure appropriate match to patients' needs.   

Knowledge, Skills, and Abilities Required:

Work requires knowledge of psychiatric disorders, diagnoses, and therapies and basic computer usage skills - acquired through completion of a bachelor's degree in a related mental health care field, such as Social Work, Nursing, Psychology, or a Rehabilitation Services specialty.  For internal candidates, high school education plus an additional 5 years of relevant experience (2 years of work must be within SPHS) may be considered a substitute for a bachelor’s degree. Position requires knowledge of psychiatric terminology, discharge and aftercare planning methods, and community referral resources - acquired through six to twelve months of relevant field or work experience in a psychiatric setting.
