Discharge Planner
The Discharge Planner will provide appropriate and timely discharge planning from the nursing center by effectively coordinating care, providing education and resources, and advocating for patients. The Discharge Planner serves as a vital link between healthcare providers, patients, and their support networks, facilitating the planning and implementation of comprehensive discharge plans.   Position Highlights
*Identifies patient/resident discharge goals at admission and documents initial discharge plan and involves the patient/resident and family throughout the process. *Works with patient/resident, family members/significant and interdisciplinary care team through care planning and utilization management during the course of the patient/resident's stay to identify strengths and needs to ensure an appropriate discharge plan is formulated. *Establishes relationships and maintains contact and referral flow with preferred providers and community-based agencies/services for discharge planning. *Serves as a liaison between post discharge care providers and the nursing center.   Benefits

*Competent in values, ethics, and theoretical perspectives; assessment; intervention; aging services, programs, and policies. Capable of building and maintaining strong relationships with members of the interprofessional team. Demonstrated excellent communication and interview skills.
