The Discharge Planner for the Norfolk County Sheriff’s Office shall follow the Department’s Chain of Command policy and work within Administration under the direction of the Director of Reentry Services. The Discharge Planner shall be responsible for the planning, development, and facilitation of inmate insurance and post-release appointment needs.
STATEMENT OF RESPONSIBILITIES:
· Shall obtain and continuously hold the Certified Application Counselor (CAC) status for MA Health
· Shall oversee any and all duties related to documentation, record maintenance and coordination of the MA Health system, while assisting with out-of-state insurance questions within 30 days of an inmate’s release, as necessary.
· Shall create and follow up on all post-release appointments including PCP, Behavioral Health, and MOUD
· Shall develop and maintain relationships with H.O.P.E. staff to assist when clients need additional insurance and appointment support.
· Shall create relationships with outside agencies and community stakeholders to increase referral potential and log all contacts
· Shall act as liaison to community physical, mental and behavioral providers and maintain frequent on-going contact with providers and/or re-entry services.
· Shall ensure client is fully prepared, considering external factors, to leave custody including completion of all appropriate paperwork, and follow-up medical/mental/ behavioral services and appointments are scheduled.
· Shall ensure statistics and reports are provided as required
· Shall be knowledgeable of and responsible for complying with ACA standards, policies and procedures of the Norfolk County Sheriff’s Office.
· Shall perform any and all responsibilities as assigned by the Sheriff or his designee
· Able to flex working hours to meet institutional needs and program coordination
· And shall perform other duties as assigned.
