Apply Today! **Job Summary and Qualifications** We are currently seeking a Discharge Planner to join our team. As our Discharge Planner you will work under the direction of the Licensed Behavioral Health Therapist, coordinate the discharge planning process to ensure patients no longer meeting acute care criteria are safely discharged and transitioned to the appropriate level of care. Your tasks and responsibilities may include: + You will oversee effective discharge planning through coordination with physicians and case management. + You will ensure staff provides appropriate discharge instructions, information, explanation, and referral to appropriate community resources. + You will evaluate patients' readiness for discharge and identifies post discharge care needs through collaboration with the patient, family and members of the interdisciplinary team. + You will develop and maintain effective relationships with appropriate community resources, post-acute care facilities and medical-equipment providers to support patient care needs post discharge. + You will provide guidance and counsel to patients and families regarding post discharge care plans, placement options and equipment needs. Provides assistance in resolving clinical, psychosocial and financial barriers. Qualifications: + Bachelor's degree in human services related field (Psychology, Social Work, Mental Health or Counseling) + One year experience in discharge planning and/or utilization management + Current **BLS** (Basic Life Support) required within 30 days of hire + Current **CPI** (Crisis Prevention Intervention) required within 60 days of hire + Critical thinking, service excellence and good interpersonal communication skills, ability to read/comprehend written instructions, ability to follow verbal instructions, PC skills **What qualifications you will need:** + Basic Cardiac Life Support must be obtained within 30 days of employment start date + Nonviolent Crisis Intervention must be obtained within 60 days of employment start date + Other-Fingerprint Required + Bachelors Degree For over 40 years, HCA Florida South Tampa Hospital has been providing comprehensive healthcare services.
