Job Description
Reporting to the HCC/DCP Manager, the Home Care Coordinator (HCC) Hospital Discharge Planner II (RN) is a licensed nurse with a thorough understanding of available home care services and products, eligibility requirements, and resources available in the community. This role acts as a discharge planning resource to hospital staff involved in home care planning and facilitates continuity of care for patients transitioning to home-based services. The position serves as the on-site home-based care expert.
ResponsibilitiesServe as a discharge planning resource for hospital staff involved in home care planning.Facilitate continuity of care for patients transitioning to home-based services.Provide expertise on available home care services and products, eligibility requirements, and community resources.Act as the on-site home-based care expert.Essential SkillsDischarge planning, particularly in home care.Basic Life Support (BLS) certification.Experience with EPIC software.Active RN License in Maryland (MD).Additional Skills & Qualifications2+ years of experience in home care/discharge planning.MD RN licensure.BLS certification.Proficiency with EPIC software.Work Environment
The work schedule is Monday to Friday, from 8:30 AM to 5:00 PM at Anne Arundel Medical Center (AAMC). There are no weekend, holiday, on-call, or floating requirements.
Pay and Benefits
The pay range for this position is $42.00 - $45.00/hr.
