Discharge Planner (Community and Social Services) Clifton Springs, NY (Onsite) Full-Time $17.00 - $18.00/Hour Apply Now (https://flacra.jobs.net/apply/jqz7l26wnhzhgz3rhw0?ipath=CR¬ify=true&siteid=cbnsv) Job Details Job Title:Discharge Planner Location:Finger Lakes Region, NY Employment Type:Full-Time Job Summary: In alignment with FLACRA’s mission, vision, and values, theACC Discharge Plannerplays a critical role in coordinating care transitions for clients, ensuring they receive appropriate post-discharge support and referrals. This position involves close collaboration with doctors, nurses, counselors, financial entitlement coordinators, and legal entities to facilitate comprehensive and individualized discharge planning, reducing recidivism and promoting long-term recovery. Minimum Qualifications: + High School Diploma or equivalent (Associates Degree preferred). + Valid NYS Driver’s License. + Experience in behavioral health, substance use disorder treatment, or a related field preferred. Job Requirements: Key Responsibilities: Discharge Planning & Coordination: + Engage clients in post-treatment planning within 24 hours of admission, assessing their needs and preferences. + Gather and review client treatment history, discharge summaries, and counselor recommendations to determine appropriate referrals. + Conduct case conferences with counselors, nurses, and financial entitlement coordinators to ensure a seamless referral process. + Present treatment team recommendations to clients and collaboratively develop a list of referral options. + Facilitate communication between clients, referral sources, and external programs to confirm placements and coordinate admissions. + Provide regular updates to the ACC treatment team regarding referral progress and discharge timelines. + Conduct follow-up outreach post-discharge to ensure continuity of care and assess client engagement with treatment services. Client & Family Engagement: + Conduct family meetings and obtain collateral information to support treatment and discharge planning. + Provide psycho-educational group sessions and therapeutic activities in alignment with client treatment plans. + Serve as an advocate for clients, addressing barriers to care and ensuring access to necessary resources. Documentation & Compliance: + Maintain accurate and up-to-date records of all referrals, discharge plans, and client interactions. + Ensure all documentation meets agency, OASAS, and regulatory standards. + Participate in team meetings and provide regular updates on client progress and discharge outcomes. Required Skills & Competencies: + Strong communication skills– Ability to interact effectively with clients, families, staff, and external providers. + Cultural competency– Experience working with diverse populations and understanding of culturally responsive care. + Problem-solving & time management– Ability to multi-task, prioritize duties, and manage crisis situations calmly and effectively. + Knowledge of addiction treatment & behavioral health systems– Familiarity with substance use disorder treatment, recovery resources, and referral processes. + Proficiency in electronic records (EHR)– Ability to document client information accurately and efficiently.
