Responsibilities

At Dignity Health Chandler Regional Medical Center we deliver humankindness through a wide range of health social and support services in the heart of Chandler Arizona. As part of our team you will provide clinical expertise support and oversight that helps ensure our patients receive exceptional care. The Discharge RN contacts discharged patients at home to ensure that they received effective discharge instructions. 

Responsibilities:

Contacts patients discharged to home to ask a series of designated questions; ensure that patients received appropriate and effective discharge instructionsOffers information based on any questions posed by the patient.If unable to answer, elevates the questions to the appropriate parties -- Charge Nurse of the discharging unit; Clinical Supervisor or Director.Documents in patient records appropriately.Utilizes physician and community resources as needed.

Qualifications

Minimum:

Experience and Education

Two (2) years of Nursing experienceADN, Diploma, BSN or Foreign Equivalent Current AZ State RN License in good standing.
