Discharge Planning and Social Work Assistant

Lillington Health and Rehabilitation Center in Lillington, NC is seeking a qualified individual to join our growing team as the Discharge Planning & Social Work Assistant.

As the Discharge Planning & Social Work Assistant, you’ll play a crucial role in providing Care Beyond Compare by ensuring seamless transitions for our patients when they discharge from our facility back into the community. You’ll also collaborate closely with the Director of Discharge Planning & Social Work in addressing the emotional, social, and practical needs of our patients and helping patients and their families access resources they need for a fulfilling stay and safe discharge.

What you'll do

· Advocate for residents’ rights and preferences, ensuring that discharge plans align with their goals for care and community integration.

· Collaborate closely with interdisciplinary teams to develop and facilitate individualized discharge plans that promote patient well-being and autonomy.

· Coordinate post-discharge services, including home health care, community resources, and follow-up appointments, to support patients’ successful transition back into the community.

· Assist patients in accessing and utilizing community resources, such as transportation services, support groups, and recreational activities.

· Provide emotional support and companionship to patients, particularly those experiencing loneliness, anxiety, or depression.

· Other tasks to support the successful operation of the healthcare center.

What you need

· Bachelor’s degree in social work, psychology, counseling, criminal justice, or other human services related field.

· Experience in post-acute social work, discharge planning, and patient relations.

· Proficient with computer programs including Microsoft Office Suite (Word, Excel, etc.)

· Excellent communication and interpersonal skills.

· Highly organized and detail oriented.

· Ability to work independently or as part of a team.
