Mountain View Hospital is looking for a Discharge Planning Assistant to join our team!
JOB SUMMARY:
The discharge planning assistant is responsible for providing accurate and timely administrative and technical support to the
discharge planning and utilization management teams, helping ensure a positive patient experience, effective coordination of
care, utilization review processes, and compliance with both hospital policy and CMS guidelines.

DUTIES AND RESPONSIBILITIES:
1. Works in close collaboration with and provides support to Discharge Planners, Social Workers and Utilization Reviewers in all aspects of case management and utilization management functions, which may include activities such as addressing authorization requests and referrals, verification of patient information, benefit review, and initiation of Medicaid application for self-pay patients.
2. Prepares and faxes necessary documentation/information to facilitate authorization and referrals to community partners, including insurances, DME
providers, home health agencies, skilled nursing facilities, Idaho Infant & Toddler Program, etc. Follows up on authorizations and referrals by phone to ensure receipt and ensure all necessary information and documentation was provided. Receives and documents authorization requests. Contacts insurance companies, payers, medical groups to acquire authorizations.
3. Coordinates, generates, and tracks both incoming and outgoing correspondence and faxes. Receives calls, faxes, and portal submissions regarding authorization requests, process inquiries, and eligibility verifications, coordinating and communicating with providers and payers for plan members to facilitate receipt of information, and/or records for prompt review and response.
4. Organizes and maintains discharge records, including date of discharge and disposition, i.e. home with no needs, home with home health, home with hospice, etc.
5. Documents all relevant information in the electronic medical record according to policy and department guidelines. Assists department leadership in data management. Runs scheduled and ad hoc reports, as requested.
6. Identifies potential issues in coordination or reimbursement of care, notifies appropriate personnel, and assists in resolution efforts.
7. Performs job duties and coordinates with other members of the healthcare team to ensure appropriate compliance with state, federal, HIPAA, and Joint Commission regulations and requirements, including identification of patients requiring Important Message from Medicare letters and Medicare Outpatient and Observation Notices, and following procedures associated with above documentation.
8. Other duties and responsibilities as assigned.
