Communicates discharge recommendations of the clinical team to patient, care givers and/or interdisciplinary team.
Responsibilities:
Participates in care planning and communicates information between patient and team. Documents plan in medical record. Gathers and arranges the necessary materials to enable patient discharge. Orders supplies/equipment for patient specific needs as identified by clinical team. Performs administrative and clerical duties in support of unit/department.
Education, License, & Cert
High school diploma.
Associate Degree desired.
Related and significant experience in the health care industry may be considered.
Experience
Experience working with the public and professional staff in the medical field is desired. Must have telephone etiquette, computers, and knowledge of electronic medical records.
Skills: Strong Computer skills. Must possess excellent telephone and ability to interact with patients, family, and caregivers.
Must have the ability to work well under pressure and maintain composure with a positive, professional attitude to interact appropriately with hospital staff, patients, families, and community agency representatives. Must be able to work independently, prioritize work and multitask. Competent verbal and written communication skills.
Essential Functions
Understands both the importance and the processes necessary to ensure patient specific continuity of care during his/her transition to a lower level of care. Interviews patients and/or families to assist the Case Manager with the identification of potential barriers to discharge, including but not limited to, financial, emotional, inadequate support systems, and/or other patient-specific barriers. Will communicate relevant patient specific findings or concerns to the Case Manager and assist the Case Manager in addressing applicable issues, which might impede a safe and timely transition to a lower level of care. Distributes the appropriate forms needed by Medicare or the Medicare Advantage plans to ensure compliance with regulations. Forms include Medicare Outpatient Observation Notice, Important Message from Medicare, Notice of Medicare Non-Coverage, and other forms when appropriate. Assists Case Manager, patient, family, and physician(s) to facilitate a safe and effective discharge plan that meets the needs of each patient. Such duties include but are not limited to sending information for scheduling outpatient provider visits, coordinating Home Health requests, ordering Durable Medical Equipment, securing Short Term Rehab/Long Term Placement. Organizes, prioritizes, and implements daily tasks related to the time access of member care. When delays are encountered, seek timely intervention from the Support Staff, Medical Director, or Designee to facilitate care. These duties include, but are not limited to scheduling diagnostic tests, ordering DME, gathering lab results, securing physician orders for upcoming procedures, etc. Assists Case Manager, the hospital discharge planner, family, and physician(s) to facilitate a safe effective discharge plan that meets the needs of each patient. Such duties include but are not limited to faxing information for scheduling outpatient specialty consultations, coordinating Home Health requests, ordering and follow up with Durable Medical Equipment (DME) In the absence of the Case Manager, will act as their Designee and assist the hospital and physician(s) with facilitating timely and safe discharges and with any relevant continuity of care needs. Will assist with the duties of the Denials Coordinator when assistance is needed to provide cross coverage. Other duties as assigned within scope of responsibility.Other Duties
