Responsibilities:
As a Discharge Planning Specialist, you will be responsible for ensuring that appropriate discharge plans are in place prior to hospital discharge and appropriate clinical services are coordinated for the member, to support the successful transition in the community setting and reduce the risk of readmissions.
In addition you will be responsible for participating in inpatient calls, discharge ready calls with the RBHA, staffing, and providing support to clinical team in identifying and developing clinically appropriate discharge plans.
