Discharge Supervisor in Behavioral Health!

If you are a behavioral health professional who is interested in joining a growing team of professionals and are ready for a new opportunity, please apply today! **Job Summary** : The Discharge Supervisor is responsible for overseeing all aspects of discharge for patients receiving services in Crisis, Inpatient, or Transition Point programs. The Discharge Supervisor attends 4-hour staffings and daily huddles with the multidisciplinary team to identify any potential discharge barriers and provide feedback on current discharge plan. The Discharge Supervisor ensures the discharge transition plan accurately reflects the discharge plan including placement name, address, outgoing referral and follow up appointments.
