Please call or text Marlene for additional info @ (769) 990-6167. **Pay** $58,240-$68,000 (max) (depending on experience) **Shift** - Monday-Friday: 9am-5pm - Flexibility needed in case of an emergency. **Qualifications** _Required:_ - Bachelor's Degree in Behavioral Health or similar. - 1-3 years of Behavioral Health and Crisis experience in a Behavioral Health setting. - Fingerprint clearing card. - Arizona Driver's License. - Clean driving record. _Preferred:_ - Master's Degree in Behavioral Health or similar. - Experience having direct reports. - Residential or Inpatient facility experience. - Title 36 and/or CPI experience. - Familiar with the resources in the area for the homeless, mentally ill, and drug addiction population. _Ideal Candidate:_ - Seeking a candidate with extensive experience in patient discharge processes. This individual should understand the importance of being thorough during this process and ensure that patients have everything they need to succeed after discharge. - We are looking for someone who is passionate about supporting individuals with mental illness and substance abuse issues. From the time of admission, they are involved in discharge planning, ensuring that family members, providers, and other support systems are actively included. The Discharge Supervisor participates in 4-hour staffing meetings and daily team huddles with the multidisciplinary team to identify any discharge barriers and provide input on the discharge plan. They ensure that the discharge transition plan accurately details the placement information, outgoing referrals, and follow-up appointments.
