Starting Salary: $52,000/year Position Summary: This position oversees and manages the authorization and utilization of services delivered for families determined by the Department of Children and Families to be at high or very high risk. The Diversion Specialist c o o r d i n a te s a n d sche du l e s F a m ily T e a m C o n feren c es to p r o mo t e healthy, stable families with the goal of preventing further involvement with the system of care and keeping families intact. Technical/Functional Expectations: Authorization Coordination-Essential Function: This is strongly recommended to ensure timely delivery of appropriate services while managing the budget allocations and identifying alternative community resources as well as other funding streams in order to maximize resources and adhere to policies and procedures. Responsible for utilization and monitoring flexible funding and service coordination in order to meet client needs. Care Planning Resources-Essential Function: Access 3rd party, Medicaid and community resources and ensures inclusion in care planning in order to maximize resources. Crisis Interventions-Essential Function: Provide crisis intervention linkages, referrals through consultation to ensure family support and stabilization in order to follow-up with emergency FTC for continued authorization. Ensures interim safety plan and follow up development in order to protect clients and agency's best interests. Facilitate FTC's-Essential Function: This is strongly recommended in order to effectively bring a diverse team of professionals, family and community resources together to provide support to the family in order to develop strategies and identify needs to address their issues. Ability to use judgement and a solution focused style to address differing points of view to achieve consensus within the FTC process. Ensure that the family voice drives the process and that decisions and actions have support of the group. Service Utilization UM/Wrap-Essential Function: Manages the daily service utilization log and submits to C.A.R.E.S. FSS Supervisor for necessary authorization to monitor costs and track other sources of services and funding. Coordinates with providers to ensure service delivery in timely manner and to ensure needs are being addressed as detailed in the individualized care plans. in concert with family members and informal and natural supports. Other duties as required: Job performance requires fulfilling other incidental or related duties as assigned, assisting and training others, and performing duties of higher rated positions from time to time for developmental purposes. Business Experience: Valid Driver's License - Essential: This position also requires a valid Florida driver’s license in order to travel throughout the County. Computer skills (Microsoft and Web based application): This position uses Microsoft and a web-based application for daily entry of case activity and the utilization management of service authorizations. 2+ years of Team Facilitation Skills with At Risk Families: This position works collaboratively with both informal and formal professional supports and must demonstrate the ability to effectively partner with collaterals and service providers. Min 1 year knowledge Child Welfare Experience: This position requires Florida Child Protection Certification. Educational and Experience Requirements: Bachelor’s Degree required. Master’s Degree in Human Services Related field preferred. 2 to 5 years related experience required. 5 to 8 years related experience preferred.
