PROGRAM: The Doula Project, per diem
POSITION: Case Manager
Summary of Position: Per Diem - Develops and /or delivers ongoing community-based assessment, treatment and supports to a person (and their family) whom is pregnant and/or parenting up to one year postpartum experiencing a severe emotional distress, increased stress, and trauma.
Specific Duties & Responsibilities:
+ Develops and / or delivers services to assist clients in gaining access to needed medical, social, educational, vocational, housing, financial and other services.
+ Maintain contact with other professional service providers for the purpose of case review or consultation regarding services to a specific client.
+ Develop treatment plans to deliver, or provide for the delivery of, services and interventionist identified by the treatment team.
+ Assist in developing natural support networks for people whom are pregnant and parenting experiencing a severe emotional disturbance and their families.
+ Assist program participants/families in accessing community resources.
+ Coordinate and/or participate in ongoing treatment team meetings.
+ Work cooperatively with local hospitals, OBS/midwives, medication-assisted therapy programs, therapists, social rehabilitative services and other agencies providing services to participants experiencing a severe emotional disturbance and their families
+ Attend staff meetings at WCMH as required.
+ Be or become Adult and infant CPR certified.
+ Willing to learn crisis de-escalation and passive restraint techniques.
+ Problem solves and manage crisis.
+ Maintain on-going communication with service providers.
+ Be or become a certified birth doula, providing birth and postpartum doula supports.
+ Ability to be on call a minimum of 10 days/month. This includes answering calls outside and attending births outside of office hours according to the on-call schedule.
+ Knowledge of maternal health outcomes, social determinants of health, and equity.
+ Expectation of ongoing trainings as provided by agency/program.
+ Other duties as assigned by supervisor.

Qualifications:
Education:
+ Bachelor’s Degree in human services or related field required.
+ Certified Birth and/or postpartum doula required or upon hiring immediately start training.
+ Certified Childbirth Educator preferred.
Experience:
+ Two years of human service work with people whom are pregnant and parenting preferred.
+ Experience providing direct instruction and therapeutic services to patients with history of trauma.
+ Possession of a valid driver’s license, excellent driving record and access to a safe, reliable, insured vehicle are required.
+ Able to walk and stand for prolonged periods of time.
+ Ability to lift and carry up to 50 pounds.
