than children adults and their families receive services from the organizationJob Summarybull Provides informal training and direct care to clients based on person centeredplans individual needsrequests and daily household operationsResponsibilitiesbull Supervises trains andor assists client in accordance with the person centered plan including personal care recreation and reinforcement of individual habilitation objectivesbull Assures clients personal property and adaptive equipment is clean and in good order including wheel chair clothing and living space Plans schedulesand transports clients to work appointments and community activitiesbull Documents routine medications and treatments as prescribed by physiciansandor health care professionalsbull Assists individuals with meal preparation and encourages good nutritionalhabits and adherence to prescribed dietsbull Effective oral and written electronic communication skillsbull Adapts positively to a fast paced changing environmentbull Maintains required licenses certifications and trainingsbull Assists in all duties essential to the operational needs of sitebull Completes all related paperwork or mandatory
